THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

I. Our commitment to your privacy

At GynJess your health information is personal, and we are committed to protecting it. We create a record of the care you receive in order to provide quality treatment and to comply with legal requirements. This notice applies to every record this practice generates, whether your care is delivered in person at our clinic in Corvallis or by telehealth.

We are required by law to maintain the privacy of your Protected Health Information (PHI), to give you this description of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

VI. Patients under 18

In some circumstances Oregon law allows a person under 18 to consent to their own health care — including reproductive health care such as contraception and the testing or treatment of sexually transmitted infections. Where a minor patient has lawfully consented to their own care, that patient holds the rights described in this notice for that care.

Whether we may share information about that care with a parent or guardian depends on Oregon law and on our professional judgment. We discuss confidentiality directly with every patient under 18 at the start of care, so that expectations are clear before anything is shared.

VII. Complaints

If you believe your privacy rights have been violated, you may file a complaint with this practice, with the Secretary of the U.S. Department of Health and Human Services, or with both. You may do both, and you do not have to complain to us first.

To complain to this practice, contact our Privacy Officer in writing at 2358 NW Kings Blvd #202, Corvallis, OR 97330, by fax at (877) 644-4599, or through a secure message in the patient portal. Please describe what happened and how we can reach you. We will look into every complaint we receive.

To complain to the federal government, write to the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201, or file a complaint through the Office for Civil Rights section of hhs.gov.

You will not be retaliated against, penalized, or denied care for filing a complaint.

Please do not send health information by email. Ordinary email is not a secure channel, and we do not use it for health information. Use the patient portal or our secure fax line instead, and we will respond the same way.

IX. Changes to this notice

We reserve the right to change our privacy practices and the terms of this notice, and to make the revised notice effective for health information we already hold as well as information we receive in the future. The current notice is always posted at gynjess.com and in the patient portal, with its effective date shown. You may request a paper copy at any time.

We are required to make a good-faith effort to obtain this acknowledgment. If a patient declines or is unable to sign, that is documented above and care is not affected. Retain this record for six years (45 CFR § 164.530(j)).
