GynJess offers care two ways: in person at our clinic in Corvallis and by telehealth anywhere in Oregon. Some policies below apply to one or the other; where that is the case, it is stated.

VI. If you have Medicare

Jessica has formally opted out of Medicare. Medicare will not pay for her services, and neither you nor GynJess may submit a claim for them. If you have Medicare or a Medicare Advantage plan, you will sign a short private contract before your first visit acknowledging that you are choosing to receive care privately and to pay out of pocket. That contract is never signed while you require emergency or urgent care.

Because no claim is filed, no superbill is issued for these visits, and a Medigap or supplemental policy will generally not reimburse either. Labs, imaging, and prescriptions Jessica orders are usually still processed under your Medicare coverage in the ordinary way.

VII. Acknowledgment

By signing below, I certify that I have read, understood, and agree to these practice policies. I certify that I am an authorized user of the credit card on file, and I understand this authorization remains in effect until I cancel it in writing.
