How to have records from a previous provider sent to GynJess — and what we actually need.

Start here: your records belong to you, but the copies stay with the organization where you were seen — not with the clinician who saw you. Even if you saw Jessica before, only you can authorize their release.

What to ask for

Requesting the complete chart means a longer wait. These are the parts that change what we do at your visit:

Send it to us

Fax is what most releasing offices prefer, and it is secure. Send to (877) 644-4599, attention GynJess — Jessica Bell, PA-C, 2358 NW Kings Blvd #202, Corvallis, OR 97330.

If they give the records to you instead, upload them through the patient portal rather than emailing them.

How long it takes

Federal law gives an organization up to 30 days to respond, though many are faster. Request them at least two weeks before your visit.

A reasonable fee may be charged for copies. Requesting only what you need usually keeps that near nothing.

Do not wait on records to be seen

Records help, but they are not required. Bring what you remember, your medications, and any results you have. We can start, and add the rest when it arrives.

If a request is refused or ignored

You have a legal right to your own records. If a request is denied or unanswered past 30 days, ask for the reason in writing and for their privacy officer. Tell us too — a clinician-to-clinician request sometimes moves faster.
